Does Coffee Cause Osteoporosis or Bone Loss?

Coffee has not been proven to cause osteoporosis. Caffeine can slightly reduce the amount of calcium your body absorbs, and some studies link very high coffee intake to lower bone density. But most research finds this effect is small and can be offset by getting enough calcium in your diet. The picture changes for people who already drink a lot of coffee and don’t get enough calcium.

Warning Signs That Deserve a Doctor’s Attention

Some signs point to bone problems that need medical evaluation, not just a change in coffee habit. Talk to a doctor promptly if you notice:

  • A broken bone from a minor fall or bump that shouldn’t have caused a fracture
  • Sudden or worsening back pain, especially in a person over 50
  • Noticeable loss of height over time
  • A stooped or hunched posture that’s new for you

These signs can happen with or without a coffee habit. They’re worth reporting to a health care provider regardless of how much caffeine you drink.

How Caffeine Affects Calcium: The Basic Mechanism

Research on caffeine and bone comes down to two main effects. First, caffeine can modestly reduce the intestine’s ability to absorb calcium from food. Second, high doses of caffeine can increase the amount of calcium the kidneys excrete in urine over the following hours.

A widely cited review found that the effect of caffeine on calcium absorption is small enough to be offset by roughly one to two tablespoons of milk. The review also noted that studies linking caffeine to bone problems were done in people who weren’t getting the recommended amount of calcium in the first place. In people who do get enough calcium, this review found no clear evidence that caffeine harms bone health.

That doesn’t mean caffeine is risk-free for everyone. It means calcium intake appears to be the bigger variable.

What Coffee-Specific Studies Show

Individual studies on coffee and bone density have produced mixed results, which is why this remains a research area rather than a settled fact.

  • Large meta-analysis: A 2023 analysis pooling 20 studies and more than 500,000 people looked at coffee and tea intake alongside bone mineral density and hip fracture risk. Findings across the pooled studies were not uniform, reflecting the extent to which study design and population differences affect results.
  • Older postmenopausal women: One study found that caffeine intake above 300 milligrams per day, roughly the amount in 18 ounces of brewed coffee, was associated with faster spinal bone loss in elderly postmenopausal women, particularly those with a specific genetic variant involved in vitamin D processing.
  • Older men vs. women: A Swedish cohort study of adults around age 72 found that men who drank 4 or more cups of coffee per day had lower bone mineral density at the hip than lighter coffee drinkers. The same pattern didn’t show up in women in that study. Men who metabolized caffeine quickly, as indicated by genetic testing, appeared to be at higher risk in this specific dataset.
  • Long-term tracking in older women: A 10-year study of postmenopausal women found no clear overall link between coffee and hip or spine bone density, but suggested that drinking more than five cups a day might be associated with lower bone density, with the effect varying depending on alcohol intake and other factors.

Observational Links Are Not Proof of Cause

Almost all of the coffee-and-bone research is observational. Researchers track what people already drink and eat, then look for patterns in bone density years later. This kind of study can show a correlation, but it can’t prove that coffee itself caused a change in bone health.

People who drink a lot of coffee often differ from lighter drinkers in other ways that also affect bone health, including:

  • Lower average calcium intake, sometimes because coffee or tea replaces milk in the diet
  • Higher alcohol use, which independently affects bone density
  • Smoking history
  • Overall diet quality and body weight

Researchers try to adjust for these factors statistically, but it’s difficult to fully separate coffee’s effect from the habits that often accompany heavy coffee drinking.

Who Should Pay Closer Attention

The research suggests the calcium-and-caffeine relationship matters most for people who are already at higher risk for bone loss, including:

  • Postmenopausal women, since estrogen decline speeds up bone loss on its own
  • Adults over 65, particularly those not meeting daily calcium and vitamin D targets
  • People who already have low bone density or an osteoporosis diagnosis
  • People whose coffee or tea intake regularly replaces dairy or other calcium sources rather than adding to a calcium-adequate diet

If you fall into one of these groups, a conversation with your doctor about your overall diet and bone health is more useful than trying to interpret single studies on your own.

A Practical Calcium and Coffee Checklist

Use this as a starting point for a conversation with your doctor or a registered dietitian, not as a substitute for one:

  • Know your daily coffee and total caffeine intake, not just cup count, since serving sizes vary widely
  • Check whether your diet includes calcium-rich foods like dairy, calcium-set tofu, sardines with bones, or fortified plant milk
  • Add a small amount of milk to coffee if calcium intake is a concern, since research suggests even a modest amount can offset caffeine’s effect on absorption
  • Note whether you drink coffee instead of, or in addition to, calcium-containing beverages
  • Ask your doctor whether bone density screening applies to you based on your age and risk factors
  • Bring up any recent fractures, height loss, or back pain at your next visit

What Screening Looks Like

Bone density testing isn’t something you do because of coffee habits alone. According to the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), the U.S. Preventive Services Task Force recommends bone density screening for women over age 65 and for younger women with factors that increase their risk of osteoporosis. The agency notes that there isn’t yet enough evidence to make a formal screening recommendation for men.

The standard test is a DXA scan, which measures bone mineral density at the hip and spine using low-dose x-rays. Evidence Limits

A few limits are worth keeping in mind when reading coffee-and-bone research:

  • Most studies are observational, so they show patterns, not proof of cause
  • Results differ by age, sex, and genetic differences in how people process caffeine
  • Definitions of “high” coffee intake vary between studies, making comparisons difficult
  • Few studies isolate coffee from other lifestyle factors that also affect bone density
  • Much of the research focuses on older postmenopausal women, so results may not apply the same way to younger adults or men

Frequently Asked Questions

Does decaf coffee affect bone health the same way?

Most of the research on caffeine and calcium absorption focuses on caffeine itself, not on other coffee compounds. Decaf coffee contains only a small amount of caffeine, so it’s less likely to have a meaningful effect on calcium absorption, though it hasn’t been studied as extensively as regular coffee.

How much coffee is considered “high intake” in these studies?

This varies by study. Some research flagged effects starting around four cups a day, while other studies looked at thresholds above five cups a day. There isn’t one universal cutoff, which is part of why findings across studies don’t always agree.

Can adding milk to coffee protect my bones?

Research suggests that a small amount of milk, around one to two tablespoons, can offset caffeine’s modest effect on calcium absorption in that cup. Milk itself is also a source of dietary calcium, which matters more overall than caffeine’s absorption effect.

Should I stop drinking coffee if I have osteoporosis?

This isn’t something to decide from an article. Osteoporosis treatment plans are individualized, and your doctor can tell you whether your specific caffeine intake matters given your diet, medications, and bone density results.
